This is the Special Request Form to be used by any player that is requesting to be on a certain team, with a certain other player and or coach. The form is only a request, and every effort will be made to honor those requests, but SUSC cannot guarantee that they will be. The requests must be received prior to the evaluation or team splitting and also the posting of any roster in the particular division you are in.
